Our team helps identify non-medical services that meet the individual needs of children from prenatal through age 17 and their families. When a family is referred to Help Me Grow, they speak directly with a Family Care Coordinator who has access to the most current resources in Orange County. After referrals are made, a referral letter is sent both to the family and your office to be included in the child’s medical record, keeping you informed of the supports provided. Care coordination continues until the family is successfully connected, and an outcome letter is shared with you to summarize the results.
